Below are answers to some questions our patients may have. Our aim is provide you with the best care possible so we make it our job to give you as much information as we can. If there are any additional questions you have that cannot be answered through our website, please do not hesitate to contact us.
How do I access my medical records?
The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. If you wish to request access to the records held by the practice, please contact our reception staff on 02476 961373. You can also request to access your clinical records via Patient Access.
How do I order a prescription?
If you have a long-term illness you may have certain medications on a repeat prescription. You can manage these via your online Patient Access account or by visiting reception. For more information on prescriptions please visit the 'Manage Your Care' page. Please allow 48 hours for us to process your request.
How do I book an appointment?
You can book an appointment with a clinician via your online Patient Access account or by calling or visiting reception. We make every effort to keep to appointment times but there may be occasions where they overrun. Please book one appointment per person and inform reception or check in electronically when you arrive. Please let us know if you cannot attend your appointment so that someone else can use your slot. You're more than welcome to bring someone with you to your appointment and if you would like a chaperone, please ask your Doctor.
How can I get help in an emergency?
A doctor is contactable during the normal working weekday using the surgery telephone number: 02476961373.
NHS Direct
In the case of urgent need when the practice is closed, please call NHS Direct on 111. The NHS Direct operators will take details about your query then assign a member of their clinical team to call you back and discuss your query in more detail.
The ambulance service
If your query is related directly to a life-threatening or emergency situation, you should telephone 999 immediately and ask for an ambulance.
How do I obtain my test results?
When you take your test you will be told how long it will take before the results are returned to the practice. To obtain your results, please telephone the surgery on 02476961373. Please be prepared to identify yourself, saying what the test was and when it was carried out.
We cannot always give full results over the phone. If the result requires further discussion, we will offer you an appointment. Due to our confidentiality and data protection policies, we will only give results to the person who has had the test.

How can I provide feedback?

How can I request a telephone consultation?
To request a telephone consultation with a doctor or nurse, please telephone the surgery on 02476 961373 before 12 noon and a receptionist will arrange for a doctor or nurse to call you back. You must be prepared to tell the receptionist the reason for your call.
If for any reason you will be unable to take the call, please notify the surgery as soon as possible.
How can I request a home visit?
We ask our patients to visit the surgery if at all possible, as our doctors can see several patients at the practice in the time it takes to make a single home visit. Home visits are only offered to patients who are unable to get to the surgery for medical reasons, and whose condition is medically urgent.
Babies and children can usually be brought to the surgery even when ill and will always be fitted in quickly if necessary.
To request a home visit, please:
Call the surgery on 02476 961373 before 10.30am.
Be prepared to tell the receptionist about your condition so we can prioritise the most urgent cases and arrange the most appropriate course of action.
Ensure the surgery has up to date details of your address and telephone number.
How can I keep healthy?
You can keep healthy by:
Not smoking or getting help to stop
Drinking no more than 14 units (female) or 21 units (male) of alcohol per week
Taking regular exercise
Eating a healthy diet
Seeing your dentist regularly
Following your doctor’s advice when taking any prescribed medicines
Keeping any appointments at the surgery or at the hospital.
Many patients will see a doctor or nurse regularly but others will not. Patients over 75 years who have not seen a doctor or nurse at all within the last 12 months, or patients between 17 and 75 years who have not seen a doctor or nurse at all within the last three years are encouraged to request a consultation for a review of their health.

How can I obtain a Fitness for Work note?
The DWP has issued strict guidelines for doctors to follow when issuing Fitness for Work notes (formerly known as ‘sick notes’).
If the illness is:
Less than 3 days: your employer should not require a Fitness for Work note
Between 3 and 7 days: you should hand your employer a "self certificate" (SC1). You do not need to see the doctor for this
More than 7 days: You will need a Fitness for Work note and must see the doctor to get one. It is only at 7 days that you need a doctors’ note. If you require a note covering illness when you saw a different doctor (e.g. casualty) the doctor at the surgery must be in receipt of written evidence that you saw another doctor.
Fitness for Work notes needed before 7 days illness will incur a charge that should be borne by your employer.

How can I register as a carer?
It is important to let us know if you are a carer for one of our patients or if you are one of our patients who cares for somebody else.
Sometimes when people are looking after someone they can get tired or frustrated and would like to know that there is someone outside the family who might understand and can help them. If we know that you are a main carer, we can offer you that help.
Not only can we take greater care of you, but we also need to know who is supporting our patients. The information you give us will be added to the notes of the person you are caring for, so we can contact you quickly in an emergency or if we have any other concerns. Also if you have concerns we know we can talk to you, as the main carer, with their permission.
To notify us that you are supporting one of our patients, please speak to one of the receptionists or contact Jaspreet Dhillon on 075 2808 0414. She also holds drop in sessions every Friday at the surgery.
